Position Summary:
We have an exciting opportunity to join our team as a Operations Manager.

In this role, the successful candidate leads a team of administrative support staff who assist the Care Management/Social Work departments. This role ensures timely, accurate, and compliant execution of key administrative workflows that enable safe, efficient transitions of care to sub-acute care providers (e.g., skilled nursing facilities), pre-authorization requests, and comprehensive transfer documentation. The Manager partners closely with Social Work, Care Managers, physicians, and post-acute providers to remove barriers to discharge, support length of stay reduction goals, and strengthen post-acute partnerships to reduce avoidable readmissions.

Job Responsibilities:

  • Directly supervise and coach Care Management Assistants, including scheduling, coverage planning, performance management, and daily work prioritization.
  • Establish clear standards of work (SOPs), service-level expectations, and quality checks for referral, authorization, and handoff documentation processes.
  • Participate in huddles to align priorities with Care Managers/Social Workers on high-impact patients (e.g., complex discharges, pending SNF placement, authorization delays).
  • Develop onboarding, training, and competency validation for CMAs (EMR workflows, payer processes, documentation standards, privacy requirements).
  • Oversee the end-to-end administrative process for sub-acute referrals, including initiating placement searches, assembling clinical packets, and ensuring accurate submission to facilities.
  • Ensure timely initiation and tracking of pre-authorization requests, including follow-up, escalation of delays, and documentation of payer communications.
  • Maintain robust tracking tools/dashboards (e.g., pending authorizations, facility responses, bed offers, barriers) and ensure real-time updates for the care team.
  • Identify recurring delays and implement process improvements
  • Partner with interdisciplinary teams to proactively identify discharge barriers and coordinate administrative actions to mitigate them (e.g., incomplete documentation, missing clinical information, authorization stalls).
  • Support hospital LOS reduction initiatives by streamlining referral/authorization processes and ensuring early initiation of post-acute planning activities.
  • Serve as an operational liaison with skilled nursing facilities and other post-acute providers to improve responsiveness, and mutual expectations.
  • Standardize and monitor the completeness of clinical handoff information sent to post-acute providers (e.g., medication lists, isolation status, wounds, therapies, dialysis, follow-up needs).
  • Support reduction of readmissions by ensuring accurate transmission of key clinical and logistical information needed for a safe transition of care.
  • Ensure workflows align with hospital policy, payer requirements, and privacy/regulatory standards (HIPAA, documentation integrity).
  • Monitor quality metrics (e.g., referral turnaround time, packet completeness, authorization cycle time, avoidable delays) and drive corrective action plans.
  • Participate in audits and continuous improvement activities related to transitions of care and utilization review support.

Minimum Qualifications:
To qualify you must have a Bachelor's degree
5+ year of people management, team lead, or supervisory experience (or strong equivalent leadership experience)
Working knowledge of post-acute care settings, payer authorization processes, and discharge planning workflows

Qualified candidates must be able to effectively communicate with all levels of the organization.
