At Compass Oncology, the Social Worker provides comprehensive, patient-centered support to individuals and their families throughout their care journey. Under general supervision, this role delivers a broad range of social work services, including assessing needs and connecting patients with community resources that address emotional, physical, social, and financial concerns.

Working collaboratively with the care team, the Social Worker plays a key role in enhancing patient well-being and access to supportive services.

Salary:

**Final offer amounts are determined by a review of the candidate's applicable experience and qualifications, as well as, internal employee equity considerations.

Jobe Scope:
Under general supervision, provides generalist social work duties to include knowledge of and referrals to agencies and organizations
offering emotional, physical, social, and financial services to patients and families. Supports and adheres to the US Oncology Compliance
Program, to include the Code of Ethics and Business Standards.


Responsibilities

-Provides information about & helps coordinate access to resources and services for patients, families, and/or caregivers, making
appropriate referrals as needed.
-Completes patient forms when necessary (i.e. patient disability forms, indigent drug program, etc.).
-May administer psychosocial needs assessments, distress and depression screens to patients and families, and then forward the
assessments needing evaluation to the appropriate, identified clinicians
-Develops and maintains working relationships with community agencies, health care organizations, and other providers to promote a broad
base of support for patients and families.
-Acts as a patient/family advocate in negotiating the health care system.
-Relays identified patient and family needs to medical team members.
-Documents all tasks in EMR accurately, appropriately, and timely.

-Demonstrates knowledge of age-specific differences in the patient population that include late adolescence, adulthood, middle age, and the
elderly. This includes physiological, psychological, and developmental differences unique to each group


Qualifications

Education:

  • Bachelor's degree in social work from a school or program accredited by the Council on Social Work Education (CSWE).

Experience/Training:

  • 0-2 years' experience preferred, working as a bachelor's-prepared social worker in a healthcare setting, oncology experience desired.
  • Appropriate state licensure or certification required for bachelors-level social work.
