BHCOR-Cor Rehab

Full-Time | 1st Shift

BHCOR-Rehab Program I Corbin, KY

The Rehab Program is the Operations side of the Rehab Unit. Positions in this department include discharge planning, case management, admissions, and therapeutic recreation.

Responsible for the development, organization, implementation, and review of all activities related to the rehabilitation of patient admission process. This includes conducting on-site visits and/or reviewing faxed information to determine the eligibility of patient referrals. Regular visits to local healthcare facilities and centers are required to support referral development and marketing efforts. The role requires effective communication and collaboration with patients and families, the Medical Director, referring physicians, the business office, and all members of the Rehab team. Maintains up-to-date records on all aspects of admissions and referrals. The position also involves verifying insurance coverage and obtaining pre-certification when needed. All employees are expected to consistently monitor and adhere to safety responsibilities and initiatives.

Description:

  • Evaluates and coordinates admissions for inpatient rehabilitation, including patient assessments, insurance authorization, and communication with referral sources, physicians, and interdisciplinary teams.

  • Facilitates admission processes by ensuring required documentation is complete, managing referral data, and providing education on rehabilitation criteria and regulatory requirements.

  • Manages rehabilitation bed availability through collaboration with nursing, case management, and patient flow teams to optimize patient placement and throughput.

  • Assesses rehabilitation candidates, develops preliminary discharge plans, identifies patient goals, and determines appropriateness for admission.

  • Monitors admission and bed management processes, identifies trends and barriers, and implements process improvement initiatives using performance improvement methodologies.

  • Serves as a liaison among referral sources, physicians, patients, families, and internal teams to improve communication, efficiency, and patient outcomes.

  • Maintains compliance with organizational policies, safety standards, infection control practices, emergency preparedness requirements, and regulatory guidelines.

  • Participates in required training, safety programs, and quality initiatives while supporting Baptist Health's mission, values, and culture of safety.

Requirements:

  • Degree in Occupational Therapy, Physical Therapy, Speech Language Therapy, or Nursing required.

  • Current Kentucky RN, PT, OT, or SLT license required.

  • Two years of related clinical experience required.

  • Certified Rehabilitation Registered Nurse (CRRN) preferred.

  • American Heart Association Basic Life Support (BLS) certification required within 90 days of hire.
