Job Description The Community Health Worker provides advanced outreach, care coordination, and patient education to address social determinants of health and connect individuals to clinical and community resources. This role supports patient engagement, benefits enrollment, barrier reduction, and continuity of care across settings including the emergency department, prenatal/postpartum, and post-discharge populations. In addition, this position includes mentoring junior staff and supporting program improvement efforts. Essential Functions

Salary Range: Minimum 24.97/hour - Maximum 41.22/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
