Austin Oaks Hospital (a UHS facility): POSITION SUMMARY: The Branches Clinical Care Coordinator serves as the first point of contact for individuals and families seeking behavioral health services through or outpatient programs. This role is responsible for answering incoming calls, assessing needs, determining the most appropriate level of care, scheduling outpatient assessments, and coordinating the intake process. The Clinical Care Coordinator also provides tours of the outpatient program and completes intake assessments to ensure a smooth transition into services. Experience working with people with co-occurring psychiatric and substance use disorders. Thorough understanding of the step down process familiarity with involved departments. Thorough understanding of the inpatient work-flowprocess, as it pertains to case management and discharge planning.
