Overview
Rutgers University Behavioral Health Care ( UBHC), established in 1971, offers a full continuum of evidence based behavioral health and addiction services for children, adolescents, adults, and seniors throughout New Jersey. UBHC's 1,060 experienced behavioral health professionals and support staff are dedicated to treatment, prevention, and education. UBHC, one of the largest providers of behavioral health care in the country, has a budget of $260 million and has 15 sites throughout New Jersey. Services are readily accessible and include: inpatient, outpatient, partial hospitalization, screening, crisis stabilization, family/caregiver support, community outreach and case management, supportive housing, supported employment, prevention and consultation, employee assistance programs, and a licensed therapeutic school from preschool through high school. Specialty services include the New Jersey suicide prevention helpline and peer help lines for police, veterans, active military, teachers, mothers of special needs children and child protective service workers. In FY2016, UBHC treated 16,199 consumers, had 24,502 admissions, and touched the lives of 19,441 individual callers through peer support. In addition, UBHC is the primary mental health training resource for the New Jersey departments of Human Services, Children and Families, and Corrections, delivering 16,000 trainings each year.
Posting Summary
Rutgers, The State University of New Jersey is seeking a Community Coordinator for the Outpatient in Monmouth Junction within Rutgers University Behavioral Health Care.

The primary purpose of the Community Coordinator is to serve as a trusted liaison between the community and CCBHC, promoting access to health and social services, improving health outcomes, and supporting individuals and families in navigating complex systems of care. The Community Coordinator provides culturally responsive outreach, education, care coordination, and advocacy to reduce barriers to services and advance health equity. The Community Coordinator plays a critical role in addressing social determinants of health, supporting transitions of care, promoting recovery-oriented practices, and ensuring continuity of services consistent with person-centered treatment planning.

Among the key duties of this position are the following:

  • Conducts community outreach to identify individuals in need of services.
  • Builds trusting relationships with individuals served, families, and community partners.
  • Provides education on health topics, prevention strategies, and available resources.
  • Participates in community events, health fairs, and stakeholder meetings.
  • Connects individuals and families to healthcare, public health programs, and social services.
  • Promotes healthcare linkage by assisting with appointment scheduling.
  • Assists to transport and accompany clients to healthcare, public health programs, and social service-related appointments as deemed necessary to ensure successful engagement and positive outcomes.
  • Supports individuals served by ensuring they understand their treatment plans and rights.
  • Shares community insights to improve program effectiveness.
  • Advocates on behalf of individuals served to reduce systemic barriers.

Minimum Education and Experience
  • Four (4) years of experience in related behavioral health, community services, or human services field or Bachelor's Degree in related Mental Health field, social or human services field.
Certifications/Licenses

  • Possess certification as a Community Health Worker or receive the certification within 1 year.
  • Applicants possessing a degree in Social Work must have an active New Jersey certification/licensure as required by the State Board of Social Work.
  • Valid NJ Driver's license.

  • Strong communication, organizational, and time management skills.

Equipment Utilized
Physical Demands and Work Environment
PHYSICAL DEMANDS;
Standing, sitting, walking, talking or hearing. No special vision requirements.
Required to travel to various community locations throughout the state. Primarily community-based, it requires home visits and travel within the service area. Combination of fieldwork and office-based responsibilities.
WORK ENVIRONMENT:
Office environment. Moderate Noise. The work environment includes both office based services and community outreach. Individual wellness checks and crisis intervention required. Subject to unpredictable situations, patient crises, to include behavioral, medical and psychiatric. Occasionally exposed to patients exhibiting assaultive behaviors. Exposure to blood borne pathogens that requires use of personal protective equipment.
Special Conditions
  • Schedule includes up to 2 evenings per week and occasional weekends to support client engagement.
