DESCRIPTION OF WORK

As an Aging Care Management Supervisor 1, you will be responsible for all activities provided by the Care Management/OPTIONS Unit of the agency while also managing the Person-Centered Counseling (PCC) program. The work of the unit includes Care Management, OPTIONS, Care Giver Support Programs, Human Services Development Funds (HFDS), as well as some informational and referral services. Your duties will entail reviewing unit referrals and assigning referrals to care managers, ensuring that each referral is comprised of detailed records and is handled with care and individualized analysis. You will monitor employee progress and complete annual performance evaluations. You will further coordinate training activities and provide in-depth training to new employees surrounding unit policies and regulations, in addition to providing valuable information regarding agency services and community resources for the population served.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Three years of experience in public or private social work and a Bachelor's Degree; or
  • Any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.

  • Hiring preference for this vacancy may be given to candidates who live within Lehigh County. If no eligible candidates who live within Lehigh County apply for this position, candidates who reside in other counties may be considered.

  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
