Summary:

The Discharge Planner is responsible for negotiating, procuring, and coordinating appropriate services and resources needed by patients after the acute inpatient phase. This role is capable of performing non-complex discharge planning tasks, such as returning patients to nursing homes, ordering durable medical equipment (DME), coordinating Home Health services, and faxing referrals to placements.

Responsibilities:


  • Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
  • Assists with appropriate interventions, working collaboratively with all healthcare team members regarding the need for discharge planning and available options.
  • Provides education as needed to enhance the healthcare team's knowledge of case management services.
  • Coordinates discharge plans to ensure the complete and expedient movement of patients across the continuum of care.
  • Fosters positive professional relationships with contacts at various agencies and facilities of different levels of care.
  • Completes documentation related to consults, the status of pending placements, and discharge disposition of EMR.
  • Collaborates to identify discharge needs, including facilities, services, and DME equipment.
  • Participates in care planning preferences in assigned areas or assigned patient populations.
  • Provides support to social workers, counselors, and care managers.
  • Completes documentation for post-acute facilities, including chart copying as assigned.
  • Communicates confidently and effectively with all levels of hospital associates and physicians.
  • Possess excellent written and verbal skills.
  • Performs other duties as assigned.

Requirements:

Education/Skills


  • Bachelor's degree preferred.

Experience


  • A minimum of two years of experience in a medical setting is preferred.
