Responsibilities

Case Manager- Behavioral Health

Horizon Health is seeking a PRN case manager to work at CarolinaEast on the inpatient Behavioral health unit.

The Case Manager is responsible for assisting the administration of Horizon Health/Crossroads IP Program. Included in these responsibilities are assisting: the development/implementation of a comprehensive community relations program and strategic planning.

Responsibilities:

  • Fulfills treatment team requests for patient referrals.
  • Researches and collects community-based resource information.
  • Coordinates/communicates with providers and mid-levels to increase treatment effectiveness.
  • Coordinates necessary care with family members and outside resources.
  • Reviews filings to ensure paperwork is complete.
  • Schedules outpatient providers discharge appointments for patients.
  • Advocates for patients and communicates with providers in community.
  • Audits IP files in a timely manner and as needed.
  • Enters inquiries for the IP program along with the CQI data, Basis-32, patient demographics, and community contact.
  • Contributes to the achievement of program annual performance goals at the contracted level.
  • Observes safety and security procedures and reports potentially unsafe conditions.
  • Other duties as assigned.

Qualifications

  • A Bachelor's Degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing or related field which includes a practicum in a health or human service agency
  • Graduate of an accredited therapy program
  • Minimum of one year experience in a similar position and/or industry
