Compensation

Salary Range: $52.93 - $81.58 / hour

The pay range above represents the lowest possible rate for the position and the highest possible rate. Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role. Most often, a newly hired employee will be placed below the midpoint of the range.

Responsibilities

Position Overview:

The RN Emergency Department (ED) Case Manager works with patients and families admitted into the Emergency Department, along with the healthcare team in the collaborative development of a plan for discharge and arrangement any needed post-acute care services. The position assists with gathering data to ensure that patient care provided meets approved admission criteria, continued stay, and appropriate level of care. RN ED CM I requires six (6) months of recent full-time acute care experience in an acute care hospital. RN ED CM II requires one (1) year of case management experience.



Qualifications

Notice to Applicants:
The position of RN ED Case Manager is open at two (2) tiers: I and II. Placement into a specific tier will be determined based on the candidate's qualifications, experience, and skills. The pay for this role will correspond to the assigned tier and ranges from $52.93 to $81.58 per hour, depending on qualifications and years of experience. We encourage candidates with varying levels of expertise to apply.

Minimum Qualifications:

    Graduate of an accredited school of registered nursing
  • Current CA Registered Nurse (RN) License
  • RN ED CM I requires six (6) months of recent, full-time acute care experience in an acute care hospital
  • RN ED CM II requires one (1) year of case management experience

Preferred Qualifications:

  • Bachelors of Science in Nursing (BSN)
  • Bilingual in Spanish
  • One (1) year of experience in Case Management and strong discharge planning skills
  • One (1) year of experience with InterQual
  • One (1) year of experience with Allscripts and Meditech

Essential Functions and Responsibilities:

  1. Acts as a liaison between physicians, nursing, other hospital departments, community agencies, health p.ans and patients/families.
  2. Responsible for discharge and transitional care planning. Identifies and assures patient continuum of care needs are met.
  3. Involves and educates patient/families regarding discharge planning and community resources.
  4. Collaborates with appropriate community resources on behalf of the patient.
  5. Refers to Social Services when appropriate.
  6. Refers quality of care issues according to policy and procedure.
  7. Reports suspected or actual abuse.
  8. Participates in Process Improvement activities.
  9. Performs other duties as assigned.
