Pay Transparency:

The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey.

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

Will be working in the community with consumers with severe and persistent mental illness in assisting them with individual service coordination, training in daily living skills, residential counseling (if applicable) , support services, crisis intervention services, education and facilitating proper medication self- administration techniques (if applicable, and health care monitoring and oversight services.)

Qualifications:

Required:

  • Must have a BSW/BA in Social Work, Psychotherapy, or related field
  • A minimum of 1 year experience in mental health related field with adolescent/adult population

Certification/Licensure:

  • Must have a current driver's license issued in the driver's state of residence without restrictions and with all necessary endorsements appropriate to the class of vehicle being operated
  • Crisis Intervention Training
  • BLS / CPR Certified through American Heart Association

Knowledge, Skills, Abilities:

  • Must be computer literate
  • Excellent communication and interpersonal skills
  • Organizational ability
  • Clinical skills

Scheduling Requirements

  • Monday to Friday
  • 8:00 am - 4:00 pm
  • 40 hours/week
  • Full Time

Essential Functions:

Ability to be a Member of Interdisciplinary Team:
* Participates in case conferences, staff meetings, individual supervisions and case presentations, and onsite/off-site trainings as needed.
* Serves as a broker between the needs and perspective of the consumer and the goals of the treatment system; may include educating treatment providers and advocating on behalf of the consumer.
* Works cooperatively and respectfully while demonstrating the ability to compromise with co-workers.
* Effectively communicates case information to all team members as indicated.
* Understands role in interdisciplinary team.
* Regularly seeks timely and appropriate assistance from management, and other resources.
* Acts as a model for professional behavior.

Documentation:
* Completes Individualized Rehabilitation Plans with consumers in a timely basis, per program's policies and procedures and consults with treatment team regarding initial and ongoing assessments.
* Demonstrates the ability to clearly write comprehensive progress notes ; progress notes that reflect the wealth of information obtained, assessment and plan within prescribed timeframes and as per program's policies and procedures.
* Keeps accurate and updated to date record of information/documentation in consumer's charts.

Counseling Skills: Individual, Group, Family and Crisis Intervention
* Establishes and maintains supportive and therapeutic relationship with consumers.
* Exhibits a positive and professional demeanor with his/her/their consumers, characterized by empathy, compassion, warmth, calmness and respect.
* Demonstrates the ability to use a wide range of verbal interventions, consistent with the model: clarify, reflect, redirect, interpret, reframe, confront, etc.; with appropriate timing and clinical justification.
* Demonstrates the ability to assist consumers in understanding and coping with stressors that trigger potential deterioration.
* Able to verbalize knowledge as to when a change in the level of care is warranted.
* Demonstrates proficiency in teaching consumers how to improve daily functioning in order to improve treatment outcomes, this includes but is not limited to educating consumers on activities of daily living, medication, social skills, financial budgeting, etc.
* Demonstrates personal responsibility to maintain competence in Evidence based Treatment Modalities (i.e practice of individual and family counseling tools, motivational interviewing tools, CBT tools) through psycho-educational activities and informal learning experiences/activities.
* Can articulate a basic knowledge of crisis intervention principles including an understanding of his/her/their purpose and role.
* Demonstrates the ability to apply intervention skills to the de-escalation of crisis situations, when warranted and communicate anticipated crisis to management in timely manner for further interventions if needed.
* Can articulate an understanding of the need for debriefing consumers, families, and collateral parties following a crisis; can describe an example of this practice, when warranted.
* Consistently demonstrates the ability to remain composed and professional in a crisis situation.

Case Management Skills:
* Assists clients in obtaining necessary documentation for application of benefits or additional assistance.
* Identifies client's needs and implements service linkage and case management skills to support clients in their wellness goals and treatment plans.
* Demonstrates proficiency in monitoring consumer's status in the community/Group Home, including assessing level of personal care and psychiatric status.
* Verbalizes an understanding that resources such as employment, housing, and similar services play an important role in community stabilization; is able to assist the consumer in making steps towards obtaining such supports when applicable.
* When encountering consumer resistance is able to reassess consumer's strengths and preferences; utilizes these effectively to motivate the consumer toward a resolution of the impasse.
* Verbalizes an accurate understanding of privacy and confidentiality guidelines as these applies to field work; appropriately seeks out supervisory guidance when considering potential breaches of confidentiality (e.g., duty to warn situations)
* Maintains an appropriate awareness of safety considerations when conducting work.
* Reflects a positive manner with consumers. Reflects an ability to show empathy and provide support; communication is client-centered and reflects the client's desires and needs.
* Provides guidance, support, and direction to consumers regarding consumer's identified goals.
* Works with consumers through supervision and assistance as needed to ensure consumers are able to appropriately complete activities of daily living.
* Provides transportation and/or teaches the skills to accommodate transportation independently to and from appointments or tasks in the community as it related to identified wellness goals. Assist in facilitation of transporting consumers for their treatment team meetings and court hearings.
