Kidney Smart is a component of The Patient Education Team. The Kidney Smart program is designed to educate and empower patients and their families around understanding and managing Chronic Kidney Disease. Kidney Smart classes are no cost and available to participants across the country. The Patient Education Programs Facilitator will be instrumental in helping to drive volume of CKD participants educated to the program by conducting direct education virtually, support with core operations for the program and provide increased capacity for our field teammates across the country to increase the number of participants receiving life-changing CKD education through Kidney Smart.
