Position Summary Information
| Primary Purpose |
The Diabetes Patient Care Clinics, funded by Abbott and administered through the Thomas J. Long School of Pharmacy, provide year-round, direct patient care services to the Stockton community. The Clinics focus on screening, prevention and management of diabetes, targeting individuals who are at risk for, or diagnosed with, prediabetes or diabetes. Services include health screenings and testing, patient-centered clinical services, disease-state education, medication management, and strategies to optimize medication access and affordability.

| Essential Functions |
Under direction of the Program Director and/or Program Manager of the Diabetes Care Clinics, this position provides advanced administrative and patient-facing support for clinic operations and community outreach activities. This role focuses on patient intake, scheduling, billing, documentation preparation, and customer service, operating under established procedures and guidance from program leadership. The position supports the efficient execution of clinic and outreach activities through accurate data handling and direct patient interaction.
Primary Responsibilities (75-80%):
- Perform patient data entry related to intake, registration, and billing, ensuring accuracy, timeliness, and compliance with established procedures and privacy regulations.
- Schedule and manage patient appointments, including confirmations, reminders, and rescheduling, using approved scheduling systems.
- Draft and update medication calendars and patient action plans based on standardized templates and instructions provided by pharmacists.
- Prepare and submit patient billing documentation and assist with reconciliation by tracking submitted charges and identifying discrepancies.
- Serve as a primary administrative point of contact for patients, responding to routine inquiries, providing appointment and process information, and directing non-routine issues to appropriate pharmacists.
- Maintain organized patient and administrative records in electronic and/or paper systems in accordance with program requirements.
Secondary Responsibilities (20-25%)
- Provide administrative support for community health fairs and outreach events, including basic logistics coordination as assigned (e.g., assisting with schedules, assembling materials, and preparing forms).
- Conduct on-site patient intake and data entry during community health fairs and outreach events using established processes and tools.
- Assist designated staff by preparing forms, sign-in sheets, and other routine administrative materials for community events.
Other responsibilities will be assigned, as needed, at the discretion of the Program Director or Manager of the Diabetes Care Clinics.
This position is for one year of grant funds, January 1, 2026 -12/31/2026. potentially renewable, based on the availability of funding.

| Minimum Qualifications |
- Two (2) years of post-high school education. Two (2) years of full-time experience in the same/similar position is equal to one (1) year of completed post-high school education coursework.
- Two (2) years of measurable work experience

| Preferred Qualifications |
- Bachelor's degree (B.A. or B.S.)
- Pharmacy technician license
- Experience with vaccination and medical billing
- Previous community outreach experience
- Fluency in another language
- Proficiency with Microsoft Office
- Experience and sensitivity in working with people of diverse backgrounds and cultures.
- Demonstrated experience in advancing social justice, equity, and inclusion in a university setting.
- Ability to engage and integrate culturally responsive practices and knowledge in their work.

Local travel required. Valid driver's license required. Incumbent must also be able to meet the University's fleet rules and be eligible to drive for University business. The University and its insurance carrier reserves the right to exclude applicants based on their driving record.

| Hiring Range |
$24.00 - $25.00 per hour. We consider factors such as, but not limited to, scope and responsibilities of the position, candidate's qualifications, internal equity, as well as market and organizational considerations when extending an offer. |
