Scheduled Weekly Hours: 40Lead Hospice Admission Nurse (RN) Location: La Crosse, WI FTE: 1.0 (80 hours per two weeks) Pay: Starting pay of 38.01/hour, with additional compensation based on experience Shift: 8-hour shifts, Monday-Friday 8am-4:30pm Holiday: Rotating every 3rd holiday Call Schedule: Back up on-call every 20th week for evenings and nights Join Our Dedicated Hospice Team At Emplify Health by Gundersen, hospice care is about compassion, dignity, and support for patients and their loved ones during life's most meaningful moments. We are seeking a Lead Hospice Admission Nurse (RN). As a Lead Hospice Admission Nurse, you will play a critical role in ensuring a smooth and efficient admission process for patients entering hospice care. This position is primarily office-based and serves as the first clinical point of contact when a referral is received. The Lead Admission Nurse gathers and organizes all clinical information, prepares documentation, and ensures readiness for the admission nurse to visit the patient's home. This role works closely with the Intake Coordinator, who manages non-clinical aspects of care, to provide a seamless experience for patients and families during the transition to hospice services.
