Hourly Pay Range:

$42.54 - $65.94 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

  • Hours: Monday-Friday, 8:30a-5:00p
  • Required Travel: Travel to other Endeavor locations

A Brief Overview:
The Clinical Coordinator (Post Acute and Population Health), will support staff in coordinating and managing care for Endeavor patients across post-acute and ambulatory settings. This role focuses on optimizing clinical outcomes, resource utilization, and patient care coordination across various care settings and providers. The Clinical Coordinator will assist in ensuring the delivery of high-quality care and service excellence, while contributing to the effective management of clinical resources. They will collaborate with physicians, clinical leaders, and interdisciplinary teams to ensure seamless care transitions and continuity for patients. Additionally, the Clinical Coordinator will provide support in the operational and administrative functions of care coordination across the post-acute network. This position requires strong leadership skills, clinical expertise, and a commitment to excellence in healthcare.

What you will do:

  • Support staff in coordinating and managing care for Endeavor patients across post-acute and ambulatory settings
  • Ensure the Delivery of High-Quality Care Management Services
  • Support Staff Development and Professional Growth
  • Provide Leadership to Ensure Operational Effectiveness
  • Collaborate with physicians, clinical leaders, and interdisciplinary teams to ensure seamless care transitions and continuity for patients

What you will need:

  • Education: Bachelor's degree in a clinical discipline required, Master's Degree preferred
  • Certification:
*Clinical certification, such as case management certification is preferred *Basic Life Support for Healthcare providers (AHA) or CPR/AED for the professional Rescuer (American red Cross) preferred
  • Experience: Minimum 3 years of quality management/care coordination/health care experience
  • Experience with ambulator services working with high-risk patients preferred
  • Strong background with care management, clinical social work beneficial
  • Experience working with home health and/or hospice beneficial
  • Unique or Preferred Skills: Strategic thinker with experience in a value-based care management environment; Care Management/Utilization Review experience with CMSA or ACMA certification preferred
