HealthPartners is looking to hire an RN Supervisor to join our Advanced Care Primary Care team! Position Summary: The Population Health Supervisor leads the Advanced Care Primary Care staff in partnership with the Population Health Leader and Clinic Leadership. May directly supervise Ambulatory Care Managers, Registered Nurses (RN), Social Workers (SW), Licensed Practical Nurses (LPN) and/or Clinic Support Staff (CSS), ensuring high-quality care for high and rising-risk patients. Collects and interprets data that is population-based in partnership with the value-based care analytics and population health teams. Provides and directs efforts to ensure patients receive appropriate, effective, and timely healthcare services, focusing on those with chronic or complex conditions, high utilization, and high risk. This role includes care planning, patient education, medication management, care coordination and care management, aiming to improve patient outcomes, reduce costs, and enhance patient experience within the scope of practice and HealthPartners standards of care. Works with the interdisciplinary team to improve patient outcomes in alignment with value-based care initiatives with a focus on the quadruple aim. This role is comprised of 30% supervisory responsibilities and data collection and 70% direct patient care. This role is part of an exciting new clinic. The Advanced Care Primary Care provides proactive, team-based care to HealthPartners patients with complex medical and social needs. This role will partner with interdisciplinary team for enhanced coordination, identifying social drivers of health, making connections and removing barriers. The care provided in this clinic is flexible and the team takes the "call us first" approach with a plan of care based on what matters most to the patient.
