Visit Facilitator Overview

Key responsibilities include but are not limited to:

  • RightFax: Print labels, print home care orders for yellow files, print FMLA paperwork & sort into files. Sort incoming faxes.
  • Certified Mail: Print and send certified mail when requested.
  • UMP Oncology Epic Pool Monitoring
  • Disperse information to providers first thing in the morning and fax signed forms as appropriate.
  • Authorization to Discuss Private Health Information
  • Faxing: Orders, urgent medical records and treatment plans
  • FMLA: Capture encounter as FMLA is received, complete paperwork as appropriate, send to requested parties/patient, send for scanning.
  • Disability/Handicap Forms
  • Complete 'Notification of a Deceased Patient' form and send to HIM
  • Lab entry: Forward labs not in Care Everywhere to lab entry team.
  • Prior Authorization Forms
Minimum Qualifications:
  • High School Diploma or GED
  • At least 1-year of customer service experience
  • Proficient with computer typing and comfortable with technology (computers)

Location: 909 Fulton St SE, Minneapolis, MN 55455

Hours: 0.5 FTE, 20 hours per week, daytime, weekday

Compensation:

21.20 - 30.74 USD Hourly
