Serves as a member of the Population Health leadership team for Jefferson Health with authority and accountability for the operations of Ambulatory Care Management services. Provides visionary leadership to build and maintain an environment which attracts and retains employees committed to continuing improvement in quality of services provided and being creative, innovative, and cost-effective. Participates in network-wide planning activities. Represents the institution in community, civic, and professional activities.

Job Description

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Engages in a variety of activities and programs designed to ensure care and operations reflect the system's vision, mission, and values.
  • Provides overall leadership and direction to JH and Population Health Care management services, strategies, plans, and operating performance related to areas of responsibility. This includes ambulatory, inpatient, ED, post-acute care management and navigation services.
  • Responsible for performance on utilization KPIs with in value based contracts.
  • Works closely with Enterprise Population Health team to evaluate the effectiveness of clinical programs and interventions, vendor contract returns on investment, and payer contract opportunities.
  • Maintains the Magnet philosophy to assure a professional practice environment. Articulates clearly and consistently as an advocate for patient care in all administrative and management decisions.
  • Develops/enhances vision for integrated care management services within the network while continually assessing internal and external political environments and to plan and/or adapt strategies appropriately.
  • Participates with all members of the leadership team to develop strategies and long and short-term goals for JH Population Health performance and outcomes.
  • Interprets and implements operating policies, procedures and programs established by senior management staff.
  • Maintains effective working relationships with all JH entities. This includes direct oversight of Care Management leadership, and interaction with medical staff, management peers, business partners, boards, community, and other professional associations.
  • Facilitates the development of an integrated delivery system by aligning JH objectives with those of the community medical centers and respective payers. Removes barriers to integration activities when necessary.
  • Holds staff accountable to achieve established goals, quality measures, efficiency targets, patient satisfaction targets, and patient safety standards.
  • Establishes, monitors, and evaluates patient engagement and patient care outcome measures by assuring standards of care coordination are developed, monitored, and improved. Fosters research activities to enhance care coordination outcomes, promotes continuous QI, assures regulatory and safety standards, and achieves designated customer satisfaction.
  • Administers and monitors revenue, capital, expense, and personnel budgets for care management services. Ensures operations remain within budgetary parameters for all services and effectively manages all business unit resources to yield optimal outcomes. Improves operational excellence through cost, quality, and service management
  • Maintains the appropriate culture and professional environment to assure availability of adequate and appropriate staff to provide quality service within a cost-effective environment. Overseeing the recruitment, selection, and promotion of staff; ensuring a strong professional environment for retention and delivering of a high-performance team.
  • Identifies global and specific areas for individual and group leadership development, promoting opportunities for career ladder growth and educational progression.
  • Enhances and promotes individual personal and professional development through publications, networking, and presentations.
  • Performs all other duties as assigned

Education and Experience:

  • Master's degree in nursing and/or health care required, Doctoral degree in Nursing with an administrative focus or Master's in business administration (MBA) preferred.
  • Minimum ten (10) years' experience in care coordination/disease management positions required, at least five (5) years at a division director level.

Certifications, Registration or License Requirements:

  • Active RN license in Pennsylvania.
  • Master's degree in nursing or health care related field.
  • Certification in Nursing Administration.
