Job Description Summary:


The People Concern, in partnership with the Department of Health Services (DHS) and their Housing for Health (HFH) Program, will provide Intensive Case Management Services (ICMS) and housing navigation services to persons experiencing homelessness county-wide. The Scattered Site Housing Program houses individuals throughout Los Angeles County in subsidized housing units.


Participants are linked to an ICMS case manager/housing navigator who provides intensive field-based case management and clinical services while employing a trauma-informed care and harm reduction approach to a diverse group of clients with complex trauma facing medical, mental health, and substance use barriers. Participants remain in the program for as long as they require services. Importantly, the Program Manager of this team must possess a working knowledge of the myriad subsidies available through CES and must be able to assist their staff in determining client eligibility for these subsidies according to the complex matrix of HUD, local Housing Authority, and unit-specific regulations.


The Program Manager is responsible for the daily oversight of the Team. This includes assistance with client crisis management, coordination of services, and clinical supervision to the case management team. The Program Manager serves as the primary liaison between DHS and the ICMS team, ensuring funder standards are met.



Job Description Essential Duties and Responsibilities:



  • Provides clinical and administrative supervision to Bachelor's level Case Managers and Master's level Clinical Case Managers.
  • Facilitates weekly team meetings and case consultation utilizing a trauma-informed care lens and harm reduction approach.
  • Collaborates with the Housing Department team, DHS, and community partners to develop creative interventions focused on improving the safety and well-being of our most marginalized clients.
  • Assists with client crisis management, which includes assessment for voluntary and involuntary psychiatric hospitalizations.
  • Be available to respond to emergency calls and situations on a 24-hour basis with support from a direct clinical supervisor and the Housing Department Director.
  • Ensure all documentation, charting, and data collection is complete, timely, and accurate through ongoing audits of electronic databases and chart reviews.
  • In coordination with the Director, provide budgetary oversight of program administration and ensure program revenue and expenses are within the adopted annual budget.
  • Responsible for hiring and staffing the ICMS team and ensuring staff are adequately trained and abide by all policies and procedures.
  • Ensure case managers are linking clients to appropriate services and utilizing a "Whatever It Takes" approach. Case Managers will provide assistance with locating and securing housing, connection to community resources, government and state benefits, medical, mental health, and substance use services.
  • Assist in the screening, assessment, and enrollment of clients, including orientation to program policies, resources and goals
  • Provide support and containment to the team in the event of client death and/or traumatic incident. Coordinate or provide a debriefing when appropriate.
  • Ensure that staff are completing enrollment in CES and that all individuals have a VISPDAT score entered into HMIS
  • Remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits/income assistance, transportation to appointments
  • Identify permanent housing units to move individuals into housing as quickly as possible once linked to a rental subsidy (TBV, PBV, EHV, TLS)
  • Perform other duties as assigned



Job Description Qualifications:



  • Bachelor's Degree
  • Must be license eligible or close to licensure
  • Minimum of two years' full-time experience working with the homeless, preferably those living with mental illness and/or substance addictions
  • Able to work in a high-tolerance Harm Reduction model with clients who have multiple barriers
  • Current, valid California Driver's with an acceptable driving record
  • Able to be on-call 24/7 with support from direct clinical supervisor and Housing Department Director



Job Description Preferred Qualifications:



  • Preferred MSW, MFT, LPC, or PsyD who is registered with CA BBS (supervision hours are available for this position)
  • Two years' supervisory/management experience preferred



Job Description Work Environment:



  • Field (may need to travel) and indoor office environment.
  • Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
  • On occasion walk or drive to different local sites.
  • Will necessitate working in busy and loud environments
  • Will be exposed to elements like cold, heat, dust, noise, and odor.
  • May need to bend, stoop, twist, and sit throughout the day.
