The Healthy Aging Clinic Licensed Social Worker position is located at the Midtown clinic location. The UC Davis Healthy Aging clinic offers comprehensive care for older adults, including geriatric medicine, dementia care, caregiver consultations, and psychosocial support. The interdisciplinary team of providers and staff at the Healthy Aging Clinic are committed to the health and well-being of older adults and their caregivers.
The primary role of the LCSW is to assist patients and families and caregivers to cope with the social and emotional issues and practical arrangements related to the patient's illness, as well as promoting the delivery of effective, cost-efficient, quality patient care. The LCSW delivers age-appropriate social work care to patients and their caregivers in accordance with agency policy and procedure and state and federal regulations. The LCSW serves as an integral member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of patients and families in the clinic setting. The LCSW shall adhere to and promote the ethics and values of the social work profession, using the NASW Code of Ethics as a guide to ethical decision making. The LCSW will advocate for the patient's right to self-determination, confidentiality, access to supportive services and resources, and appropriate inclusion in decision making that affects their health and well-being. The LCSW will be an active participant and work collaboratively with the multidisciplinary team by recognizing and offering services that address the biopsychosocial needs of the patient. The LCSW position requires a high level of clinical judgement, problem-solving skills, creativity, organizational and priority setting abilities, with excellent communication skills. The LCSW demonstrates the ability to fulfill these responsibilities through clinical assessment, accurate and sound judgement, and maintaining professional communication. The LCSW may be asked to participate in special projects and collect data to improve the quality of services delivered to our patients. The LCSW must be flexible as changes in technology, healthcare coverage, regulatory requirements, and our patient population may mandate changes to workflow, work standards, and work practices on an as needed basis.
The incumbent will potentially also assist with referrals from additional specialty departments not listed here, according to operational need.

Qualifications
Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position
- California Licensed Clinical Social Worker (LCSW)
- Graduation from an accredited school of social work (MSW)
- Minimum of three (3) years recent experience in social work required
- Demonstrated ability to work on a multidisciplinary team
- Must have solid psychosocial assessment skills
- Must demonstrate ability to effectively and efficiently handle demanding workload involving multiple tasks
- Must be able to master detailed and complex information regarding coordination of care
- Excellent analytical, interpersonal, and problem-solving skills
- Extensive, broad-based clinical experience
- Ability to perform in-depth chart analysis to obtain and communicate accurate information in a concise, informative manner
- Experience in identifying and analyzing care/service needs for patients
- Requires a high level of clinical judgment, problem-solving skills, creativity, organizational and priority setting abilities, with excellent communication skills
- Knowledge of care/services provided in differing levels of care in ambulatory and institutional settings
- Knowledge of community resources
- Knowledge of biopsychosocial and behavioral health treatment plans
- Ability to work with all levels of staff, both inter- and intra-departmental to create a cooperative relationship and positive working environment
- Knowledge of legal requirements and guidelines for providing clinical social work services, including mental status exams, suicide risk assessments, 5150 applications, and mandated reporting requirements
Preferred Qualifications
- Experience working with older adults/geriatrics preferred
- Experience completing advance care planning preferred
- Experience providing short-term supportive counseling, grief counseling, or similar preferred
- Experience facilitating support groups with patients/families/caregivers preferred
- Training and experience in evidence-based therapies (CBT, Behavioral Activation, Problem-Solving Therapy) preferred
