Sub-Acute Rehab Therapy Manager
Summary Manage daily operation of sub-acute rehabilitation therapy department through leadership, education and clinical supervision that promotes clinically and financially effective interdisciplinary patient-centered care. Essential Job Responsibilities The Therapy Manager is responsible for the daily operations and direction of the assigned rehab program, including clinical responsibility, communication with customers and the rehab team. The Manager is responsible for program compliance and the effective and efficient delivery of clinical services. The Therapy Manager regularly promotes and engages in teamwork, collaboration and transparency among colleagues and pursues career development through professional organizations, networking, continuing education and training opportunities. This non-exempt, full-time position reports to the VP of Sub-Acute Rehabilitation, NHA. The successful candidate operates independently, with integrity, and will be extremely organized with complete attention to detail. The candidate will be highly driven with strong management skills, follow-through and a focus on owning the outcome, as well as an ability to concentrate on the most important objective at the current moment, with a high level of adaptability. ESSENTIAL FUNCTIONS OF THE JOB
- Responsible for day-to-day management of the therapy program (including but not limited to clinical programing, financial goals/objectives, action plans, assistance with performance reviews of therapists in the program, etc.
- Must have strong working knowledge of the PDPM Payment System which is unique to SNF settings
- Strong communication in collaboration with IDT to ensure best practices in GG coding
- Must be able to lead discussions and trainings with staff therapists in the matter of GG coding from a clinical and reimbursement perspective
- Strong understanding of Medicare and Managed Care products, including authorizations for continued care practices
- Facilitates discharge planning
- Assists in creating, leading, and training clinical pathways for individualized care plans in collaboration with interdisciplinary team.
- Responsible for scheduling or facilitating daily staff coverage at the facility level.
- Responsible for accuracy of EOM process mid-month and completing EOM and ongoing accuracy checks with month end accuracy check list.
- Responsible for assisting with facility orientation with new hires.
- Responsible for facility tours and assists with interview process.
- Attend facility meetings including but not limited to the following: UR, Rehab, Falls, and QA
- Completes clinical duties as assigned by VP of Sub-Acute Rehabilitation.
- Provides marketing assistance as needed.
- Manages resources / supplies for department.
- Complete monthly audits with compliance, documentation, certification tracking, etc.
- Coordinate administrative duty completion including filing, FOM data entry, therapy room maintenance, equipment calibration, etc.
- Consistently portray the mission, vision, core values, cornerstones and professional image of Mary Free Bed, implements Rounding, and exhibits Standards of Behavior in every interaction
- Special projects and other duties as assigned.

- Hold a license to practice in the state(s) where services are being provided.
- Therapist with experience as a clinician, previous Rehab Manager experience preferred.
- Preferably have a minimum of one year of experience working in long-term care.
- Staff will provide patient care, treatment, and services within the scope of their license, certification or registration and as required by law and regulation.
- Provide the oversite for specific groups of staff, including physical therapists, speech therapists, occupational therapists, rehab techs
- Responsible for the recruitment, selection, and training of staff, and provide them ongoing mentoring, development, continuing education opportunities, and performance appraisal/feedback.
- Assure staff competence to perform job responsibilities is assessed, demonstrated, and maintained, according to regulatory and accreditation requirements and in alignment with recognized standards of care and best practice benchmarks.
- Identify risks to employee safety and take appropriate steps to reduce those risks. Maintain a physical work environment that reflects the high-quality services delivered at Mary Free Bed.
- Oversight of therapy program quality metrics/outcomes. Analyze quality measures and reimbursement data to identify opportunities for clinical and financial improvement. Implement process improvement plans as necessary to maintain high quality performance.
- Complete managed care insurance requirements of participation for continued stay authorizations daily. Complete Section O of MDS for all patients. Coordinate with MDS nurses to ensure Medicare skilled criteria requirements are satisfied with documentation completion and compliance.
- Complete month-end therapy billing
- Administrator and point of contact for therapy electronic medical record system
- Review and provide therapy documentation in the event of insurance appeal/denial
Clinical
- Establish treatment objectives, written treatment plan, and administer treatment for each patient according to the stated objectives. Re-evaluate patient's status and effects of treatment. Revise treatment plan as necessary.
- Communicate with Interdisciplinary Team to integrate therapy treatment that optimizes total patient care. Update patient care plans in real-time to maximize treatment plan and patient safety.
- Participate in planning for patient discharge by considering patient, family, community support/services, and environmental limitations. Provides patient/caregiver education for discharge planning and post discharge plan of care.
- Educational Responsibilities:
- Provides education/training of staff in the form of in-services.
- Participates in program development and/or research activities as requested.
- Serves as a resource person for patient care in areas of recognized clinical expertise.
- Serves as a Clinical Instructor for specified therapy students. Supervises, teaches, and evaluates skills of students. Communicates with participating universities regarding skill levels of students.

- Graduate of an accredited Physical Therapy, Occupational Therapy, Recreational Therapy, or Speech Language Pathology program. Will also consider PTA, COTA program graduates.
- Licensed (or eligible for licensure) in Michigan.
- Minimum 4 years work experience in a relevant clinical rehabilitation setting.
- Possesses and demonstrates expertise in a specific diagnostic area of rehabilitation.
- Possesses and maintains CPR certification to meet department/program protocol.
- The physical ability to lift, position, and transport patients with physical impairments.
- Demonstrates ability to supervise students.
Preferred Job Qualifications
- Experience with government reimbursement systems and other regulatory bodies related to service line.
- Demonstrates competence in relevant software and hardware required to meet job responsibilities.
