Description

The Children's Health Center clinic is seeking a compassionate and experienced RN to support our Pediatric Rheumatology Program.In this role, you will provide specialized nursing care to infants, children, and adolescents with autoimmune disease in the outpatient setting. Responsibilitiesinclude, but are not limited to:

  • Provide case management, including nursing services, patient teaching, coordination of medical authorization and referrals and follow-up nursing care
  • Assessing patient's regimen of care and streamline diagnostic and therapeutic treatment plans
  • Discharge planning and outpatient care coordination
  • Educate families on how to organize and administer medications, including subcutaneous injections
  • Provide care plan oversight with physician specialists and participate in parent and patient education on managing autoimmune diseases, nutrition and preventive care
  • Nurse coordinator for assigned California Children's Services (CCS) Special Care Center

Salary Range: $62.43-$92.81 Hourly

Qualifications

Required:

  • Minimum of one year of pediatric clinical experience
  • Bachelor of Science in Nursing (BSN)
  • Current valid California Registered Nurse license
  • BLS from the American Red Cross or American Heart Association
  • Ability to coordinate patient care, using a team approach
  • Ability to recognize abnormal laboratory and clinical data on a patient and take appropriate nursing action
  • Communication skills necessary for translating medical/technical material to patients and their families in a more simplified fashion so that it can be understood
  • Knowledge of professional nursing standards & procedures and ability to assess clinical situations and triage clinical matters appropriately by level of urgency
  • Demonstration of skill in executing assigned projects in an independent and responsive manner
  • Skill in data collection and analysis sufficient to evaluate and implement proper patient care and promote quality improvement efforts
  • Ability to perform effectively under conditions of fluctuating workload and/or frequent interruptions
  • Skill in establishing, organizing, and maintaining a comprehensive computerized patient database.
  • Basic computer literacy with working knowledge of Word, Excel, and PowerPoint
  • Demonstrated skills in interpersonal interactions and patient care coordination
  • Ability to work in an independent leadership role within a team of healthcare professionals.

Preferred:

  • PALS Certified
  • Bilingual (English/Spanish)
  • California Children's Services (CCS) paneled
  • Familiarity with the CCS and Medi-Cal programs
