Inova Schar Cancer Institute is looking for a dedicated Community Health Worker to join our team. This role will be Full-Time, Monday-Friday, 8:00 AM-4:30 PM (some weekend and evenings hours required)

Community Health Worker Job Responsibilities:


The Community Health Worker works closely with community health, case management care teams, and social services agencies to provide short-term care coordination and connection to resources and programs for clients to improve their health and general well-being through education and provision of coordination of care and services. Works in both clinical and community-based settings, including client's homes. Serves as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, navigation support, and advocacy. Also works in community outreach, which includes but is not limited to home visits, assessments, care coordination, and health screenings.



  • Communicates the program's purpose and impact to clients and advocates for individual and community health equity. Helps clients identify socio-economic factors affecting their health and develop management plans. Supports clients in understanding care plans and instructions. Identifies and addresses barriers to care for individuals.
  • Assists clients in accessing health services (e.g., medical home, overcoming care barriers).
  • Assists clients in homes, communities, clinics, and ER settings. Offers care, support, follow-up, and education in community settings.
  • Collects, tracks, and reports community data and benefits. Executes and supports evidence based community health programs. Contributes to departmental goals and adheres to policies, procedures, and safety standards. Ensures compliance with governmental and accreditation regulations. Documents client encounters, activities, service plans, and outcomes. Maintains comprehensive electronic client files (notes, release of information, assessments).
  • Integrates with hospital care teams to ensure client wellness and care plan progress.
  • Refers individuals to federal, state, private, or nonprofit health and human services programs. Coordinates referrals and follow-up services.
  • Serves as a liaison between communities, individuals, and healthcare organizations.
  • Distribute financial aid application forms and provide aid in completing and submitting them, addressing inquiries about the forms, and offering advice on necessary documentation for the financial aid program.
  • Monitor the progress of referred patients and communicate any denials or approvals to the appropriate team members.
  • Upon request from team members, contact patients with upcoming appointments, addressing questions about the financial aid process and explaining what to expect at Saville.
  • Offer lay navigation and support during appointments as requested and available.
  • Guide referred patients to suitable Inova resources such as social workers and financial navigators if additional barriers to care are identified.
  • Attend community events (at least 2 per quarter) to disseminate information about resources to community members.
  • Participate in the planning and execution of the annual Health Fair.
  • Provide culturally and linguistically appropriate educational materials regarding cancer screening and prevention at mobile health events and community events
  • Assist Inova patients in the Shared Decision-Making process to better understand their colon cancer screening options
  • Place orders for colon cancer screening and follow-up on results in conjunction with the community health nurse
  • Follow-up on abnormal results from the mobile health unit
  • Some travel required throughout Northern VA
  • Performs other duties as assigned.


Minimum Qualifications:



  • Education: High School or GED


Preferred Qualification:



  • Community health background and experience
  • Bilingual in Spanish (or other languages)
