Nurse Practitioner / Physician Assistant - Primary Care Float / Sub-Zero

Work Schedule:

80-100% FTE, Monday - Friday between the hours of 7:00 AM - 5:00 PM. No holidays or call. You will work 50% at a variety of primary care clinics within UW Health including Family Medicine and General Internal Medicine within Madison, WI and the surrounding regional clinics. You will also work 50% at the Sub-Zero Employer On Site Services, located in Fitchburg, WI. During the week you work at the Sub-Zero Clinic - you will have a half day on Friday.

As an Advanced Practice Provider, you'll play an integral role in our care teams by providing high-quality, cost-effective care to patients.

We are seeking a Nurse Practitioner OR Physician Assistant to:

  • Float to a variety of clinics. You will rotate one week at the UW Health Sub-Zero Employer On Site Services, located in Fitchburg, WI and one week at a variety of clinics within UW Health including Family Medicine and General Internal Medicine within Madison, WI and the surrounding regional clinics.
  • The Sub-Zero Clinic is a dedicated on-site healthcare facility at the Sub-Zero Group headquarters in Fitchburg that supports employees and their families.
  • This role provides a unique opportunity to collaborate across various locations and foster professional connections throughout the Madison area.
  • Perform annual physical exams and help keep patients up to date on health maintenance.
  • Care for the acute and chronic needs of patients from newborn to elderly.
  • While at the Sub-Zero Clinic, you will be the only advanced practice provider (APP) in the clinic, requiring the ability to make decisions independently.
  • An APP with at least two years' experience in Family Medicine, Primary Care or Internal Medicine would succeed in this role.

Education:
If Nurse Practitioner: Master's or Doctorate's degree from accredited Nursing Program.
If Physician Assistant: Bachelor's or Master's degree from accredited PA Program.

Licenses and Certifications:
Minimum - State of Wisconsin NP or PA licensure
If Nurse Practitioner*:

  • Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND
  • Active board certification as a NP in area of clinical practice. These may include the following: Family.

If Physician Assistant*:

  • Active PA license, including prescriptive authority, in state(s) of practice AND
  • Active PA board certification

All Nurse Practitioner and Physician Assistant applicants must have*: Active DEA license or ability to apply for such license prior to or upon hire
