Community Support Specialist(MYPAC) Diamond Grove Center is searching for a experienced Community Support Specialist. This position is fulltime, the hours will consist of 8:00am-5:00pm. Diamond Grove Center is a 61 bed, behavioral psychiatric hospital located in the beautiful neighborhood of Louisville, Miss. Diamond Grove Center features individual units for children, adolescents. We offer inpatient care acute care, residential treatment, as well as outpatient swervices, in Starkville, Miss. On average 1000 patients receive care from our compassionate health care team each year at Diamond Grove Center Hospital. Services are primarily community-based, and participating individuals have mental health treatment needs and need support additional to outpatient therapy services. The Community Support Specialist handles new referrals, creates service plans, reviews progress and makes determinations about closure and transitions to lower levels of care. The Community Support Specialist coordinates the flow of clinical information across agencies and providers while protecting the confidentiality of individuals served. The Community Support Specialist builds partnerships with enrolled individuals with the goal of helping them achieve wellness and autonomy.

Two (2) years' experience (internship may be included) in a psychiatric or clinical setting. And Or Two (2) years' experience working with child/adolescent population. Demonstrates leadership, communication, and interpersonal skills. Must be eligible for Department of Mental Health position certification per role requirements. Ability to communicate orally and n writing with others. Current Crisis Prevention Intervention(CPI) certification; current Cardiopulmonary Resuscitation(CPR) certification. Must be mobile throughout the facility and system entities to attend meetings. Positions requires flexible hours. |
