Community Support Specialist(MYPAC)

Diamond Grove Center is searching for a experienced Community Support Specialist. This position is fulltime, the hours will consist of 8:00am-5:00pm.

Diamond Grove Center is a 61 bed, behavioral psychiatric hospital located in the beautiful neighborhood of Louisville, Miss. Diamond Grove Center features individual units for children, adolescents. We offer inpatient care acute care, residential treatment, as well as outpatient swervices, in Starkville, Miss. On average 1000 patients receive care from our compassionate health care team each year at Diamond Grove Center Hospital. Community Support Specialist is responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and youth and family centered. Services are primarily community-based, and participating individuals have mental health treatment needs and need support additional to outpatient therapy services. The Community Support Specialist handles new referrals, creates service plans, reviews progress and makes determinations about closure and transitions to lower levels of care. The Community Support Specialist coordinates the flow of clinical information across agencies and providers while protecting the confidentiality of individuals served. The Community Support Specialist builds partnerships with enrolled individuals with the goal of helping them achieve wellness and autonomy.

  • Follows the Recovery Plan created by the team throughout the month and ensures that task completion and support efforts support the identified strategies implemented to meet underlying needs.
  • Acts as a consultant for the team regarding family-driven care and navigating the child-serving system.
  • Meets regularly with supervisor or designee for supervision and seeks further supervision when appropriate and necessary.
  • Self-monitors billing and productivity levels and gives feedback to supervisor regarding workflow and/or billing concerns.
  • Communicates timely with the Supervisor, Wraparound Facilitator, Therapist and other members of the clinical team to coordinate services to youth and families.
  • Solicits input from all team members about preferences, efficacy and barriers to the success of the ISP and uses the information gathered from the team to inform their work with the family.
  • Follows the ISP created by the team throughout the month and ensures that task completion and support efforts support the identified strategies implemented to meet underlying needs.
  • Attends all CFTMs for the families they are serving and participates actively in creating the ISP with the team as facilitated by the WF.
  • Supports families in exercising their Freedom of Choice regarding their providers and supports and empowers them to take the lead in all decisions regarding their care.
  • Can articulate the mission, vision and values of MYPAC, and the practice of high-quality peer support services compliant with Medicaid regulations for enrolled individuals.
  • Performs other duties as assigned/required by this position.

Qualifications

Must have an Associate Degree or higher.

Two (2) years' experience (internship may be included) in a psychiatric or clinical setting. And Or Two (2) years' experience working with child/adolescent population.

Demonstrates leadership, communication, and interpersonal skills.

Must be eligible for Department of Mental Health position certification per role requirements.

Ability to communicate orally and n writing with others.

Current Crisis Prevention Intervention(CPI) certification; current Cardiopulmonary Resuscitation(CPR) certification.

Must be mobile throughout the facility and system entities to attend meetings.

Positions requires flexible hours.
