Providing hospice care as part of clinician team during evening and overnight hours. This position allows the hospice nurse to visit patients and families during a time of increased need, following the plan of care, initiating new interventions, provide teaching and hands on care. The RN Evening Case Manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments. This position is part of the Massachusetts Nurses Association Bargaining Unit at Cooley Dickinson VNA & Hospice. This position reports to the Hospice Manager and operates within established organizational and departmental policies and procedures. Education Other Certificate/Diploma Nursing required or Bachelor's Degree Nursing preferred Licenses and Credentials Class D Passenger Vehicle Driver's License [State License] quired Registered Nurse [RN - State License] - Experience Minimum of one (1) year medical/surgical nursing experience required Hospice/home care experience preferred Knowledge, Skills and Abilities - Graduate of an approved, accredited school of professional nursing required. - Demonstrated excellent verbal and written communication skills required. - Demonstrated strong interpersonal, organizational, and time management skills required. - Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same including competency in pain and symptom management required. - Good driving record, and reliable transportation required. Provide end of life care for patients and their families; Monday through Friday 4:00 pm-9pm with on call from 9pm-8am Monday through Thursday. No weekend commitment.
