St. Ann's Community is currently looking for a full time Registered Nurse for the Education located on the Portland Campus. The Wound Care Nurse works with the nursing and medical teams to prevent and treat pressure ulcers in our skilled nursing communities. St. Ann's Community Offers:

Job Description:
- Weekly wound care visits with nursing in SAH (LTC and Rehab Units), SACC, and for all patients and residents with pressure, venous, arterial, and diabetic ulcers. Rashes, lesions, and surgical wounds are followed at discretion of Medical and wound care NP. Wound visits to be recorded as per electronic documentation.
- Works collaboratively with other disciplines (nursing/physical/ occupational therapy, dietary, social work) regarding wound care issues to enhance outcomes
- Work closely with outside vendors to ensure continued best practices in regard to skin/wound care products utilized. Initiate trials as indicated.
- Works in collaboration with education, nursing and medical to provide education regarding wound care products, wound prevention, detection, treatment, documentation, and expectations based on guidance in the state operations manual, and current research. The type of education will be provided based on situation.
- Works in collaboration with to coordinate mattress and cushion needs; trialing new products as appropriate, working collaboratively with vendors, PT, staff, residents, housekeeping, and purchasing.
- Research ideas for pressure reducing devices, present ideas to education, medical and nursing. Provide trials of products as appropriate and document findings to determine whether to proceed or not proceed with a product.
- Works with nursing and medical to manage wound care costs and trends (working closely the facility Part B vendor) comply with Medicare coverage criteria), intervening and educating as appropriate.
- Provide nursing units with current information regarding coverage criteria in order to minimize costs as possible; monitor appropriate usage of products
- Continue close relationships with outside vendors, ie: Smith and Nephew, Hollister, Convatec, Blackburn's, Liberty Mission Health, etc. to ensure product knowledge, efficiency, and cost control.
- Remain current in best practices, Medicare guidelines, DOH regulations, etc., via journals, educational opportunities, WCEI, Global Wound Academy, etc.
- Maintain Tx 337 and Skin Integrity and Wound Management Program aligned with current standards of practice/regulatory guidelines.
- Provide ongoing learning opportunities to staff via ongoing on the spot education.
- Will complete tracking and reporting of wounds to Quality, Medical, and Nursing, to identify trends on units; intervene as appropriate.
- Manage ordering and billing of special requests, such as rental mattresses (ie. Bariatric) negative pressure wound therapy, etc.
- Serve as a resource to medical and nursing staff.
- Maintains Skin Integrity and Wound Management resource material
- Obtains ankle -brachial index and determines and applies appropriate compression therapy as indicated
- Works collaboratively with outside providers, including derm, surgeons, vascular offices etc to coordinate patient care efforts as indicated
- Updates/ educates resident/ patient families as indicated
- Recognizes and rewards excellence in care as it relates to wound prevention/ healing
- Other duties as assigned by manager
Candidate Requirements:
- Valid RN license - required
- BLS - required
- Previous experience in Wound Care ++
