**Available to regular status employees who are scheduled to work a minimum of 24 hours. Southcoast Health is an Equal Opportunity Employer. Responsibilities Under the direction and supervision of the Director of Supportive Care Services or designee, perform a variety of duties both within and in support of Hospice and Palliative services including, but not limited to; closely collaborating with both inpatient and outpatient staff to transition patients to Hospice and Palliative Services, discussing goals of care with every patient and documenting such conversations, educating the physicians, referral sources and community about the services available within the Hospice and Palliative Care Team, acting as a liaison with referral sources and identifying the benefits of hospice services to the community and potential clients. The Conversation Nurse is also responsible for census development within the hospice program, including increasing hospice census, increasing Length of Stay and tracking referral source needs and feedback. Qualifications
Compensation: Pay rate will be determined based on level of experience. Pay Range USD $86,444.80 - USD $153,732.80 /Yr. |
