Overview

Evaluate, document and respond to written, telephone and in person requests for information contained in the legal medical record. These requests could come from other health care organizations, Secretary of State requests, attorneys, insurance companies, patients, and other interested third parties. Maintains off-site database to track clinical records stored off-site. Accurately performs tasks related to closing charts when enrollment has ended. Assists with Chart Reviews to assure accuracy and completeness
Qualifications

Certifications:

Education: High school diploma or GED.

Work Experience: Electronic medical record, Read, write and speak fluent English, Demonstrates the ability to multi-task and work in an organized, efficient, and process-oriented manner, exhibits excellent oral communication skills and the ability to interact with customers and team members with diverse education, lifestyle and back grounds in accordance to the affiliate's mission and vision. Possesses complex problem-solving skills in reasoning through work related issues, Demonstrates the ability to adapt to change, Ability to write clear and concise, grammatically correct notes and business correspondence without close supervision, Keyboarding skills required; preferred experience with MS Office suite and the ability to learn new applications, Familiarity with Health Information Exchanges and patient portals, Coordinates work to achieve maximum productivity and efficiencies with little or no supervision, Requires knowledge of health information disclosure practices, concepts, laws and regulation.


Responsibilities

Key Accountability - Release of Information
  • Responds to request for medical information from patients, extended families, insurance companies, attorneys, court orders/subpoenas, physicians and/or their office staff, other health care facilities, government, regulatory bodies, and other third parties.
  • Invoice for appropriate requestors and follows up on payments.
  • Assures receipt of a HIPAA-compliant authorization and/or request.
  • Collaborate with transcription support, legal staff, Patient Access and Privacy Officer to investigate and resolve issues related to documents, MPI, and other HIPAA requests such as accounting for disclosures.
  • Confirm receipt of valid authorization for electronic exchange and indicate compliance within the electronic health record.
  • Monitor and/or marks for correction scanned document errors
Key Accountability - Customer Service
  • Promptly answers office phone calls or assists with phone coverage.
  • Use telephone etiquette consistent with UnityPoint Health Values and Standards of Behaviors.
  • Respond to and fulfill patient walk-in record requests as required.
  • Responsible for handling and/or scanning documentation received through mail or fax as needed.
  • Ability to access and retrieve patient information from various computer systems and various types of media.
  • Clean copier/fax machines and ensure they are in proper working order and place service calls as appropriate.
  • Keep workstations and work area clean and orderly.

Compensation and Benefits

The compensation range for this position is $16.03per hour - $25.97per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
