RN Float - Manatee and Sarasota counties

Monday - Friday 8a to 5p. Manatee and Sarasota counties.

Tidewell Hospice, a member of Empath Health, is seeking an Registered Nurse to join our Float Team.

The RN Case Manager and Float RN uses the nursing processes and nurse case management strategies to coordinate care and provide nursing services for hospice patients and their families in private homes, assisted living facilities and long term care communities. The Float RN works in all of these areas to provide care as needed.

The RN Case Manager:

• Reviews and updates the care plan, physician orders, pharmacy profile and other records.
• Educates patients, family members and other caregivers about the disease process and decline, available palliative interventions, the dying process and safety practices.

Requirements:

• Applicants must be a graduate of an approved nursing program and own a current Florida RN license.
• BSN or MSN preferred.
• Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a hospice RN preferred.
