Patient Concierge

Suncoast Hospice, a member of Empath Health is currently seeking a Patient Concierge to join our team.

Schedule: M-F 9a-6p

The Patient Concierge:

• Is the first responder from our agency who is passionate about the services we offer and works directly with patient/family/authorized representative, or any other customer to insure optimal access.
• Keeps patient centered services and caring at the forefront, exceeding customer expectations and insuring best practice and flawless care in all aspects.

Position Requirements:

• Education and/or Experience: High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
• Excellent verbal and written communication skills.
• Knowledge of medical terminology.
• Computer skills.
• Knowledge of Empath Health/Suncoast Hospice programs.
• Basic knowledge of reimbursement for services.
• Working knowledge of community resources.
• Ability to listen, assess needs, and coordinate care.
• Possess appropriate license and vehicle for travel needed to perform the job responsibilities.
